1. Ask your primary care provider to send a referral to Gastroenterology Associates of Western Michigan, PLC. We accept referrals through fax: (616) 452-4142. The referral must contain demographics (including insurance card), diagnosis, and all testing and notes associated with your diagnosis. We require a referral for all patients if we have not seen you in the past 3 years.
2. Once we receive your complete referral, we will review and contact you to schedule an appointment.
